Thank you for choosing to adopt a pet! The decision to adopt a pet is an important one which we take very seriously. Please take a moment to read and carefully complete this adoption application. In order to adopt a pet, you must:

  • be at least 21 years old
  • be able to provide references and identification
  • consent to a home visit
  • pay the adoption fee (listed below)

All of our pets come spayed or neutered, up to date on vaccinations, are microchipped, heartworm negative, current on flea/tick and heartworm prevention, dewormed, and treated for any ailments found upon veterinary examination. The adoption fee covers a portion of these services, and makes it possible for us to continue rescuing animals.

An application does not obligate you to adopt, and can be transferred to any pet. You will receive an automated confirmation once your application is successfully submitted. Please understand that due to the volume of applications some animals receive, we are unable to contact all applicants. If your application is considered a good fit for the pet you are interested in, or even a different pet, a volunteer will give you a call within 7-10 days. If you would like to transfer your application to a different pet, please use the Contact Us form. Be sure to include your application number in your request.

Adoption Contract and Affirmation


am applying to adopt from Louie's Legacy Animal Rescue. I understand that this contract applies to whichever animal I select and am approved to adopt, even if not the pet listed in the application.

All animal adoption contracts approved by Louie's Legacy Animal Rescue are subject to the following terms and conditions:

The adopter agrees that the animal will be kept as a domesticated house pet. This means that dogs are kept indoors except for periods of exercise in a fenced-in area or on a leash. Cats will be kept indoors at all times unless a screened-in porch or similar-type enclosure is available.

The adopter grants Louie's Legacy Animal Rescue the right to make periodic visits to the adopter's premises for the purpose of checking on the health and general welfare of the animal and to verify that the animal has been cared for according to the terms of this agreement.

The adopter represents that all household members have agreed to the adoption of the animal and that all household members will abide by the terms of this agreement. The adopter further represents that he/she is twenty-one years of age or older.

The adopter represents that he/she has never been subject to legal action for cruelty to or neglect of animals. The adopter further represents that he/she has never owned an animal which has been confiscated by an animal control officer or humane organization for violations of state or local animal control regulations or animal adoption agreements.

If the adopter is a tenant, the adopter agrees to provide written permission from the landlord consenting to the animal adoption.

The adopter understands that most rscued animals come from shelters, where the animal may have picked up an illness or parasite. Animals are quarantined and fully vetted while in our care, and all known medical issues are disclosed, but additional veterinary attention may be needed at the owner's expense. The adopter agrees that they will not solicit Louie's Legacy Animal Rescue for coverage of future medical expenses.

If for any reason the adopter is unable to provide care to the animal, the adopter agrees to return the animal to Louie's Legacy Animal Rescue. The adopter shall not rehome the animal, offer the animal for sale, or give the animal to any third party without the prior written consent of Louie's Legacy Animal Rescue.

The adopter agrees that Louie's Legacy Animal Rescue shall have the right of immediate possession of the animal if, in the judgment of Louie's Legacy Animal Rescue, the animal is receiving inadequate care, or is being improperly housed or handled.

The adopter agrees to pay Louie's Legacy Animal Rescue the sum of $300.00 as liquidated damages in the event that the terms of this contract are breached; this liquidated damage value does not bar Louie's Legacy Animal Rescue from seeking the return of the animal by judicial process or other legal means if necessary. If legal action is instituted, the adopter agrees to pay reasonable attorney's fees and court costs.

The adopter hereby declares that he/she is aware:

(a.) That animals are different from human beings in their responses to human actions;

(b.) That the actions of animals are often unpredictable;

(c.) That the animal's behavior may change after it leaves the shelter or other environment and accustoms itself to its new environment;

(d.) That Louie's Legacy Animal Rescue makes no claims as to the temperament, health, age, breed, or mental disposition of any animal put up for adoption;

(e.) That the adopter, upon taking physical possession of the animal, hereby accepts legal guardianship and responsibility for the animal referenced in this contract and discharges Louie's Legacy Animal Rescue forever from liability for any injury or damage to any person or property caused in the future by said animal, and from any causes of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages.

(f.) That there will be NO REFUNDS OF THE ADOPTION FEE.

(g.) That the adopter has received a copy of Louie's Legacy Animal Rescue's Adoption Contract Terms (Please print for your records).

You should receive an automated confirmation of your application, in an email titled "Adoption Application #" with a confirmation number. Please note that this is not an approval, rather a confirmation that your application was received and will be reviewed for adoption. Applications being considered for adoption are contacted by phone within 7-10 days, and approval takes place only after a phone screening and reference checks.
